Multicenter phase 1-2 trial of linvoseltamab for relapsed or refractory multiple myeloma

Linvoseltamab is a BCMA x CD3 bispecific antibody. A multicenter phase 1-2 trial in the United States evaluated linvoseltamab for relapsed or refractory multiple myeloma (RRMM) with progression after at least three prior lines of therapy or triple-refractory (to a proteasome inhibitor, immunomodulatory drug, and anti-CD38 monoclonal antibody). In phase 2, linvoseltamab 200 was given once per week through week 14, followed by once every two weeks, and after 24 weeks every four weeks in patients with at least very good partial response. The treatment induced 71% overall response rate with 50% rate of complete response or better. The median duration of response was 29.4 months. The most common adverse event was cytokine release syndrome in 46% of patients (grade 3 in 0.9% of patients). The authors conclude that linvoseltamab 200 mg induced deep and durable responses in patients with RRMM, with and acceptable safety profile.
