Due to evolving treatment standards for newly diagnosed multiple myeloma (MM), many patients with be triple-class exposed after initial relapse and have poor survival. Novel therapies and combinations are therefore required to improve outcomes. The multicenter phase 1-2 ALGONQUIN trial in Canada evaluated various doses and schedules of the anti-BCMA antibody-drug conjugate belantamab mafodotin in combination with pomalidomide and dexamethasone for MM patients who are lenalidomide refractory and proteasome inhibitor exposed. The primary endpoints, including evaluating dose-limiting toxicities, establishing the RP2D, and overall response rate for patients treated at the RP2D, were met. Patients on study had a median of three previous regimens and 55.2% were triple-class refractrory. A the RP2D, the overall response rate was 85.3%, and the ≥ very good partial response rate was 75.7%. This figure shows survival outcomes. The authors conclude that belantamab mafodotin plus pomalidomide and dexamethasone induced durable responses with promising survival in relapsed MM.
