Easy and accessible biomarkers associated with response to immune checkpoint inhibition (ICI)-treated melanoma are limited. A cohort study including 1202 patients with advanced melanoma at 11 centers in The Netherlands investigated the association of artificial intelligence (AI)-detected tumor-infiltrating lymphocytes (TILs) with clinical outcomes with first-line ICI. TILs in pretreatment metastases were quantified using a melanoma-specific publicly available AI model trained on an independent data set. Higher TILs levels were associated with improved response, progression-free survival, and overall survival independent of clinical predictors.
