Molecular determinants of outcomes with pembrolizumab versus paclitaxel for gastroesophageal adenocarcinoma

In the phase 3 KEYNOTE-061 trial, pembrolizumab did not significantly improve overall survival versus paclitaxel as second-line therapy for gastric/gastroesophageal junction adenocarcinoma with PD-L1 combined positive score ≥ 1 tumors. Exporatory analysis of the study now found a strong association between tissue tumor mutational burden and efficacy of pembrolizumab but not paclitaxel. tTMB could be a significant and independent predictor beyond PD-L1 status.
