Minimal residual disease (MRD) status after initial therapy strongly predicts outcomes in newly diagnosed multiple myeloma (NDMM), yet MRD status is not used to adjust the intensity and duration of therapy. The multicenter phase 2 MASTER trial treated NDMM patients with daratumumab, carfilzomub, lenalidomide, and dexamethasone (Dara-KRD) followed by autologous hematopoietic cell transplantation and MRD-guided post-HCT intensification or cessation of therapy. This strategy led to most patients achieving MRD-negativity and ceasing therapy.
