A meta-analysis of four phase 3 trials (2320 patients) evaluated the survival impact of addition of immunotherapy to first-line chemotherapy for locally advanced or metastatic endometrial cancer. Patients in the groups with immunotherapy plus chemotherapy had significant longer progression-free survival compared with patients with chemotherapy alone (HR 0.70; 95% CI 0.62-0.790; this benefit was most pronounced among patients with MMRd/MSI-H tumors (0.33; 0.23-0.43) but was also seen among patients with MMRp/MSS tumors (0.74; 0.60-0.91). Immunotherapy plus chemotherapy versus chemotherapy alone was also associated with improved overall survival (HR 0.75; 95%-bti 0.63-0.89), although OS data maturity was low. The authors conclude that incorporation of immunotherapy into the initial treatment for locally advanced or metastatic endometrial cancer was associate with improved oncologic outcomes, especially in the MMRd/MSI-H subset.
