Matched-pair analysis of differences in prognosis and recurrence patterns between ulcerative colitis-associated and sporadic colorectal cancer

A matched-pair analysis of patients of Yokohama City University investigated prognosis of ulcerative colitis-associated colorectal cancer (UC-CRC) and sporadic colorectal cancer (S-CRC). Patients with UC-CRC underwent more invasive surgery and had longer operative times than those with S-CRC, but there were no significant differences between the groups in postoperative complications or perioperative mortality The 5-year overall survival rate was 86.5% for UC-CRC versus 88.8% for S-CRC (p=0.742); however, in stage 3 patients, 5-year OS rate was 51.4% for UC-CRC versus 83.8% for S-CRC (p=0.032). The first recurrence sites in stage 3 UC-CRC were mostly peritoneum and bones, while they were liver and pulmonary system in stage 3 S-CRC. The authors conclude that UC-CRC had a poorer prognosis than S-CRC at stage 3, and that the recurrence patterns differed between the groups.
