Long-term update of a phase 3 trial of hypofractionated, dose-escalated versus conventionally fractionated radiation for localized prostate cancer

Dr. Comron Hassanzadeh

A phase 3 study at MD Anderson Cancer Center (Houston, TX) randomized men with localized prostate cancer to hypofractionated, dose-escalated radiation (HIMRT, 72 Gy in 2.4 Gy fractions over 6 weeks) or conventionally fractionated radiation (CIMRT, 75.6 Gy in 1.8 Gy fractions over 8.4 weeks). With a median follow-up of 13.2 years, treatment failure occurred less frequently in the HIMRT group (13/111) than in the CIMRT group (22/111) although the difference did not meet statistical significance (p=0.08). There were no significant differences between the groups in distant metastasis, grade ≥ 2 GI toxicities, and grade ≥ 2 GU toxicities. HIMRT was a safe alternative to CIMRT, with shorter duration of treatment.
