The multinational phase 3 study CLL14 randomized patients with previously untreated CLL to venetoclax plus obinutuzumab (Ven-Obi) or clorambucil plus obinutuzumab (Clb-Obi). Three months after treatment completion, 40% of patients in the Ven-Obi arm versus 7% in de Clb-Obi arm had undetectable MRD levels<10-6 in peripheral blood. At median follow-up of 52.4 months, with all patients at least three years of treatment, median PFS was not reached in the Ven-Obi arm and was 36.4 months in the Clb-Obi arm (HR 0.33; p<0.0001). The authors conclude that Ven-Obi compared to Clb-Obi had superior long-term efficacy with the majority of Ven-Obi treated patients remaining in remission.
