In metastatic esophagogastric adenocarcinoma (EGA), the addition of PD-1 inhibitors to chemotherapy has improved outcomes in selected patient populations. The multicenter randomized phase 2 AIO INTEGA trial compared nivolumab plus trastuzumab combined with ipilimumab versus mFOLFOX6 chemotherapy for newly diagnosed advanced HER2-positive EGA. The chemotherapy group showed favorable survival and response compared with historical control. This was not seen in the ipilimumab group. The authors conclude that chemotherapy should not be replaced by ipilimumab in un unselected HER2-positive advanced EGA population (visual abstract).
