Despite advancements in NSCLC treatment, PD-L1 negative patients remain a therapeutic challenge. Individual participant data meta-analysis of phase 1, 2, and 3 trials found that combination therapies, particularly ICIs with chemotherapy, significantly improve objective response rate, progression-free survival, and overall survival in PD-L1 negative NSCLC. Network meta-analysis found the combination of pembrolizumab plus platinum-based chemotherapy to be most effective, achieving the highest ORR and 45% PFS improvement compared to chemotherapy alone.
