The phase 2 START-NEW-ERA trial in Italy evaluated stereotactic ablative radiotherapy for unresectable locally advanced NSCLC in patients unfit for concurrent chemoradiotherapy. After median 72 months follow-up (ragen 10-108) the 5-years PFS and OS rates were 266.% and 46.7%, respectively. No patients developed grade > 3 late toxicities. The local recurrence rate was higher in patients receiving 35 Gy compared to those receiving at least 40 Gy (p=0.037). The authors conclude that these results confirm robust and sustainable benefit of SABRT in LA-NSCLC patients unfit for concurrent chemotherapy.
