The multinational phase 3 study KEYNOTE-024 randomized patients with mNSCLC and PD-L1 tumor proportion score at least 50%, and no sensitizing EGFR or ALK alterations, to first-line pembrolizumab (n=154) or investigator’s choice of platinum-based chemotherapy (n=151). Among patients in the chemotherapy group 99 received anti-PD-(L)1 therapy upon progression. Five-year follow-up of the study showed superior OS and PFS with pembrolizumab over chemotherapy. Among patients receiving all planned pembrolizumab doses (35 cycles; two years; n=39) the five-year OS was 82.1%. Toxicity did not increase with longer treatment exposure.
