Patients with BRAFV600E-mutated metastatic non-small cell lung cancer can be treated with ICIs with or without chemotherapy or with BRAF plus MEK inhibitors. The multinational retrospective FRONT-BRAF study compared these two options as first-line therapy for these patients. The study found that first-line ICIs with or without chemotherapy were associated with improved overall survival compared with BRAF plus MEK inhibitors (HR 0.69; p=0.039), particularly among patients with a history of smoking, patients with a PD-L1 tumor proportion score ≥ 1%, patients aged ≥ 70 years, patients with TP53 co-mutations and patients without brain metastases. These findings require confimation from prospective studies.
