Epcoritamab is a CD3xCD20 bispecific antibody. The multinational EPCORE NHL-1 trial evaluated epcoritamab monotherapy for relapsed or refractory large B-cell lymphoma (R/R LBCL). Long-term (median 25.1 months) follow-up of the trial found overall response in 63.1% of patients and complete response in 40.1%. Estimated 24-months progression-free survival and overall survival rates were 27.8% and 44.6%, respectively, with improved PFS and OS among patients with complete or partial responses. Among minimal residual disease (MRD)-evaluable patients, 45.4% had MRD-negativity, which correlated with longer PFS and OS. The most common treatment-emergent adverse events were cytokine release syndrome (51.0%), pyrexia (24.8%), fatigue (24.2%), and neutropenia (23.6%). The authors conclude that these results underscore long-term benefit of epcoritamab for R/R LBCL.
