131I-metaiodobenzylguanidine (MIBG) is an active radiotherapeutic for neuroblastoma. A randomized phase 2 study from the New Approaches to Neuroblastoma Therapy (NANT) consortium compared three MIBG regimens for relapsed or refractory neuroblastoma: MIBG alone, MIBG-vincristine-irinotecan, and MIBG-vorinostat. Highest observed response rate was 32% with MIBG-vorinostat, versus 14% with either of the other two regimes. Patients with MIBG-vincristine-irinotecan had higher rates of toxicity.
