Immune checkpoint inhibitors have revolutionized the treatment of advanced melanoma; however, the majority of patients with eventually progress, and half still die of melanoma. Biomarkers of response and survival with ICIs for advanced melanoma are needed. A multinational study (1644 patients of 16 centers in Australia, United States, and Europe) resulted in validated multivariable models of response, progression-free survival, and overall survival with anti-PD-1 with or without ipilimumab for metastatic melanoma, on the basis of pretreatment clinicopathologic factors (ECOG performance status, liver and lung and brain metastases, serum LDH, blood NLR, hemoglobin, therapy, and line of treatment).
