The American College of Physicians published a new guidance statement on breast cancer screening in adult females at average risk (no personal history of breast cancer or diagnosis of a high-risk breast lesion, no genetic mutation known to increase risk, no other familial breast cancer risk syndrome, no history of high-dose radiation therapy to the chest at young age). In females aged 40 to 49 years, clinicians should discuss the females risk of breast cancer, values and preferences, and uncertainty around benefits and harms. In females aged 50 to 74 years, clinicians should use biennial mammography. In females aged 75 years or older or with a limited life expectancy, clinicians should discuss discontinuation of screening. In females with BI-RADS category C or D, clinicians should consider supplemental digital breast tomosynthesis, but not MRI or ultrasound.
