Adjuvant osimertinib for EGFR-mutated stage IB-IIIA non-small cell lung cancer: 8-year overall survival analysis from ADAURA

The multinational phase 3 ADAURA trial showed improved disease-free survival (DFS) and overall survival (OS) with adjuvant osimertinib versus placebo after resection of EGFR-mutated stage IB-IIIA non-small cell lung cancer (NSCLC). Post hoc exploratory analysis of long-term OS has now been published. A total of 682 patients were randomized to osimertinib or placebo. Updated survival data were collected from 431 patients alive at the planned final OS analysis (27 January 2023) until the current analysis (4 May 2026). In the stage II-IIIA population, 8-year OS rates were 74% with osimertinib versus 58% with placebo (HR 0.53; 95% CI 0.38-0.75), with median follow-up 92.0 versus 68.5 months. In the stage IB-IIIA population, 8-year OS rates were 79% versus 64% (HR 0.52; 95% CI 0.39-0.71). OS HRs according to EGFR ex19del and L858R mutations were 0.45 (95% CI 0.29-0.68) and 0.72; 0.46-1.11), respectively. OS benefit with osimertinib was observed across subgroups. The authors conclude that these long-term results confirm the survival benefit of adjuvant osimertinib in EGFR-mutated stage IB-IIIA NSCLC.
