Acquired RET fusions have been reported at resistance to EGFR inhibitors in EGFR-mutant non-small cell lung cancer (NSCLC). A retrospective study in five countries evaluated addition of the RET inhibitor selpercatinib to osimertinib for advanced EGFR-mutant NSCLC with an acquired RET fusion as a mechanism for osimertinib resistance. Addition of selpercatinib to osimertinib was feasible and safe, and offered clinical benefit (respons rate 50%; clinical benefit rate 83%). The authors conclude that these results support the prospective evaluation of this combination.
